Medication Update Form

This form is used to update Illuminate Academy with any medication information throughout the year. This form is a supplement to our Medication Information form that is filled out at the beginning of the instructional year (or during initial enrollment).

This form covers:
1. Dosage changes
2. Additions of new medications
3. Removal of a medication

GRANTS/FINANCIAL ASSISTANCE

TN Dept of Intellectual and Developmental Disabilities

The primary purpose of the program is to support: Families who have school-aged or younger children with severe disabilities Adults with severe disabilities who choose to live with their families Adults with severe disabilities not supported by other residential programs funded by state or federal funds Services can include but are not limited to: Respite care, day care services, home modifications, equipment, supplies, personal assistance, transportation, homemaker services, housing costs, health-related needs, nursing, and counseling. Services are flexible and responsive to families and their needs. An essential element of the Family Support Program is family and consumer involvement. Local and District Councils have been established and meet on a regular basis to oversee and provide advice on the distribution of local services.

Tennessee Caregiver Coalition

Our mission is to help primary caregivers who are caring for someone with special needs in the same home. While the TRC can help with identifying the resources for your self-care plan, you are empowered to select the appropriate programs, facilities or individuals to care for your loved one during your break.

ACT Today! Grants

ACT Today! grants are designed to provide access to individuals and families affected by Autism Spectrum Disorders. Grant payments will be made directly to pre-approved treatment providers, assessors or materials vendors.

CARE Family Grant Program

CARE Family Grant Program is mainly for helping families with medication, autism diagnosis/evaluation, therapy sessions (including speech, OT and ABA,) and autism summer camps.

Summer Scholarship Program

ASDF partners with autism organizations that offer social skills summer camps as well as equine therapy and aquatic therapy camps throughout the United States. ASDF provides partial and full scholarships to those families who need financial assistance to send their children to camp. While we cannot help everyone, we were able to send many autistic children to camp last year through our Summer Scholarship Program.

The Community Foundation of Middle Tennessee

This fund is intended to endow permanent assistance to families and children in need, such as those who are without food, clothing, medical assistance or shelter.

Easter Seals provides in- home services and support for adults who have severe physical or cognitive impairments. Adults with autism are eligible for services, including companionship/respite, homemaker/housekeeping, therapy, health services and personal care. In-home care services that preserve dignity and self-esteem as well as promote increased independence are offered. MyGOAL Inc

MyGOAL Autism Grant is a program to support families caring for individuals with Autism. The MyGOAL Autism Grant is offered once a year, applications are available at the beginning of the year and decisions are rendered in mid-July.

NAA’s Give A Voice program

The intent of NAA’s Give A Voice program is to provide communication devices to individuals with autism who are non-verbal or minimally verbal, and whose communication challenges put them at increased risk of injury or harm.

Various types of financial assistance for residents of Davidson County, including rent, utilities, and holiday assistance.  Applications (in person) accepted Mon. & Tues.  Priority given to single moms, elderly, and persons w/disabilities.

Small Steps in Speech

Small Steps in Speech is a 501(c)(3) charitable non-profit organization which provides grants on behalf of children with speech and language disorders for therapies, treatments, communicative devices, and other services aimed at improving their communication skills.

TACA Family Scholarship

The TACA Family Scholarship Fund was developed to help families who are pursuing treatment for their children with autism, but are struggling to find the funding. We understand how difficult it is to stretch the family budget. It is our desire to provide limited financial assistance to eligible families. Thanks to a generous anonymous donation and our participation in the CDFI Challenge we are able to make the following scholarship opportunities available.

United Health Care Children's Foundation

The UnitedHealthcare Children’s Foundation (UHCCF) is a 501©(3) charitable organization that provides medical grants to help children gain access to health-related services not covered, or not fully covered, by their parents’ commercial health insurance plan. Families can receive up to $5,000 annually per child ($10,000 lifetime maximum per child), and do not need to have insurance through United Healthcare to be eligible.
